Joe, a Filipino Rockssical: Play Review Essay

We were assigned to watch a stage play entitled Joe, a Filipino Rockssical last July 22, 2012 at St. Scholastica’s College, Manila. Actually, I have no interest in watching stage plays but after witnessing the presentation, my point of view changed. The play was about our national hero, Dr. Jose Rizal, but who is Rizal? For me, Rizal in this modern period is an instrument for us to love our country more despite of the dilemmas or struggles experiencing by it. Even our country had some flaws or imperfections, even the views and beliefs of others to our country were unlovely, we should still be proud and fight for it like what Rizal who’s brave as a lion did. He was also an inspiration to all youths because of his distinct attributes and his perseverance when it comes to studies. The stage play, Joe, a Filipino Rockssical is a kind of presentation wherein it was accompanied by songs and dances which is for me, an effective strategy because it can avoid boredom among the viewers. It has three chapters and used Filipino language for us to understand the play. It also used a little English and Espaà ±ol. This play was not a drama at all, it was also blended with comedy that can crack someone up and will make your jaws fall from laughter. But the most important thing is that it has a moral lesson that the viewers will realize after watching. The stage play was started through a lively song and dance. Six students of Rizal Integrated School were assigned to research and perform a stage play about our national hero, Dr. Jose Rizal, for the celebration of the 50th Foundation Day of the school. Joecas that symbolizes a man who values his craft more than anyone or anything else, is the writer and director, Joanne that symbolizes the character of having a principle and always with the truth, is the researcher, Hunter that symbolizes a character of having a principle and belief but can sacrifice it for the love of his life, is the composer of music, Bimbo that symbolizes a character that is persistent and diligent in reaching his dreams, is the choreographer, Ambo that symbolizes the character that is gleeful despite of his struggles especially when it comes to love, is the set designer, and Julia that symbolizes a character that has determination in reaching her dreams, is the fashion designer. They finished the play successfu lly although they have a little background or idea about the national hero. Ten years after, Joecas became a prominent writer. He was commissioned by the government to make and direct a stage play about the life of Rizal to be presented as a tribute for his 150th Anniversary of being a hero. Joecas together with his wife Joanne searched for his friends way back in high school to help him in the making of a stage play entitled Joe, the name that Josephine used to call Jose Rizal. Joanne became a successful researcher, Hunter became a rebel who climbed up in the mountains of Banawe, Bimbo became a DJ, Julia became a famous fashion designer, and Ambo owned a parlor. They have decided to stay at the condo of Joecas while finishing the play. During the researching and writing of the play, they learned many things about Rizal’s life. All the significant parts of the history about his live were presented. Some of these were about his love life including Josephine Bracken and Leonora, the writing of the novels that woke the consciousness of the Filipinos, the studying of Rizal abroad and here in Philippines, the exile of Rizal to Dapitan, the retraction of his ideologies to embrace again the Catholicism and to marry Josephine, and the shooting of him at Bagumbayan. The six main characters in the play also faced the different issues in their lives which are similar and compared to the life of Rizal. These consisted of the love of Joecas for Joanne and not sacrificing his love for art which is his true love, the love of Hunter for Joanne and the sacrifice of his ideologies, facing the true sexual preference of Ambo, consciousness of Bimbo about the problems of society, and Julia’s will of studying again. The conflict which is about the retraction was ended through a song and dance wherein the viewers were given the chance to decide whether Rizal signed the retraction or not. At the same time, they presented that even though Rizal signed it or not, what is important is that he had a stand about the ideologies and principles he’s fighting for and he kicked the bucket for the country. After all what happened, years passed, Joecas was given an award and honor for the stage play entitled Joe and he met his friends again during the event. The stage play was ended through a beautiful and magnificent music. In this play, many social illnesses are mentioned and shown and some of these are still occurring nowadays. In the dialogue stated by Ambo, â€Å"Palibhasa kabit ang nanay mo!†, is one of the social illnesses. It symbolizes a character of being a gossiper or spreading a speculation about a certain thing or person. It can cause a dispute or quarrel between the involved people. It can also cause disharmony or disunity. â€Å"I’m so fed up with the so called professionals, sobra na nga maningil wala pang work ethics. At sa sobrang dami ng raket, hindi marunong magcommit†¦Ã¢â‚¬  This was said by Joecas. It is one of the social illneses nowadays. Many people have too much confidence or belief in them, the belief that they are superb in terms of doing their work. Not only that, they are also asking for too much payment that is not appropriate, that’s why Philippines is not progressing. â€Å"Isipin na ang mabuting ina ay iba sa inang linalang ng prayle. Dapat palakihin ang anak na malapit baga sa larawan ng tunay na Diyos. Diyos na di nasusuhulan, Diyos na di masakim sa salapi, Diyos na ama ng lahat, na walang kinikilingan, Diyos na hindi tumataba sa dugo ng mahihirap, na hindi nagsasaya sa daing ng naruruhagi at nangbubulag ng matalinong isip.† These are the words mentioned at the back or the window of condo while Hunter and Joanne were fighting. The characteristics of God mentioned was contradictory to our government wherein it can be defined as another social illness. The scene where Ambo said that, â€Å"Hindi naman nasusukat sa sexual preference ang pagkatao at karakter. What is important is the heart and soul.† Judging a person is also one of the social illnesses acted in the play. The reason why most of the people do not believe in their talent is because they were primed by their anxiousness or shyness. The ability of a person is not measured by either physical appearance or sexual preference. In the scene where George Tauffer raped Josephine Bracken can be included as one of the social illnesses nowadays. Rape and other crimes are very rampant during these days. Sometimes, even their own fathers or stepfathers were the ones involved in this kind of situation. There are many barbarians in our country nowadays due to the lack of education or else because of drug addiction. The social illness like spreading information that is not proven or seen is very widespread nowadays. One example of this is occurring in the world of showbiz or show business. Many rumors are spreading in this industry that’s why it’s complicated. Those professionals that were taking too much charges or fees are also rampant nowadays that’s why the rich becomes richer and the poor becomes poorer. Similar to our government officials who live high on the hog, they were corrupting the taxes and other cash received from citizens for their personal use instead of giving it back in the form of basic services like education, health cares, and others. Criminals were also very rampant now and then but because police authorities like the friars before accept bribes, those culprits were not afraid to do crimes. These are some social illnesses mentioned in the play and at the same time I witnessed while watching. This stage play is not presented just to entertain but also to educate the audience. For me, having a stand in everything we do and say is the moral lesson that I have realized after watching. If we said and did something, continue it until the end as long as we know that we’re on the side of truth, although it can hurt those people that we loved the most. Another lesson derived from this play is the true love for the country. If our heroes can defend our country from opportunists and at the same time proud of it, we, the citizens of the Philippines, that have the capability to change the undesirable system of our government, can also do what they have done. It is the time for us to move and act especially the young generation that according to Dr. Jose Rizal is the hope of the nation.

Psychology Adjustment

Thomas Wisnewski Psychology of Adjustment Jurgens 6 December 2012 In my class Psychology 2101 I was asked to write about three major events that occurred during my lifetime for class. I have thought about this almost all semester and it was a constant struggle just to think of even one event that I would have liked to share. This class has shown me that almost everybody has their problems, some that cope with them well, and others that still struggle to figure a path that will lead them to happiness. I myself have personal issues that I have learned from.This paper will describe the three events and how they have impacted my life. During the paper I will do my best to put these events in chronological order. I come from a military family. My dad was the military man and my mother was from Korea. They had married when my dad was stationed in South Korea between the late 80s and early 90s. Being in a military family had resulted us in moving a lot. I have moved between many states whil e my dad was in the military, but I had actually loved it. I loved traveling between place-to-place and seeing all kind of new things.Travelling was just really interesting to me because there was always a time where I could experience something new. One of the places that I moved to and stayed for quite a while was in Colorado Springs, Colorado. I had always thought this place was an amazing. Most military brats know that making friends is not the easiest thing to do. We understand by the constant moving that we have to make new friends and lose the ones we made from a place before. This was normal, but getting older I realized that I was getting a bit tired from the constant moving and wanted to stay in one place for a while.As far as I can remember I have learned a lot living in Colorado Springs, Colorado. I learned to ride my first bike there and enjoyed the beautiful weather. In Colorado I had also met my best friend. My best friend’s name was Joshua Vialpando and we had almost done everything together. We would always have the same teachers in school and then of course we would hang outside of school constantly. The guy was always there for me when I ever had a problem and I would try my best to be there for him. Joshua had always struggled in school, but I would always catch on things a bit quicker and help him out when he needed it.This was my first real friend that I have made that I could actually see being there later in my life during adulthood. Of course as time went by it was time to pack my bags and move to a different state. For the first time in my life I realized that I did not want to move and that moving somewhere new had no appeal to me. I wanted to stay and just hang with my best friend and do whatever new activity would cross our minds. I felt like that moving somewhere new would change me somehow and that I would not be able to make a friend that was like him.I felt as if I was going to be lonely, or probably feel a bit different from everyone else. I was losing a friend and it felt like I was losing the world that I had gotten use to and I did not want to experience a new one. This first event gradually leads up to my second event when I moved to Augusta, Georgia. It took me a four-day trip by RV to make it to Augusta. I knew that by moving here I would have to start over again, new school and hopefully new friends as well. In school before I was never really made fun of or teased at all for being who I am.For the first time in my life I was being teased for being Asian American. Children would constantly come up to me and make â€Å"oriental† noises like â€Å"ching chong chong† and other terms like that. I feel as if in Colorado it was more racially diverse, while moving here to Georgia there was just simply mostly white and black. I remember my very first day of attending class in Augusta and I remember after school I had ended up crying. I did not understand why the other kids would make fun of me because I had never experience this issue before. Augusta, Georgia was the place that my dad decided to retire in.I knew from that point that this place would be my new permanent home and I would not be able to go anywhere else new again. I hated the new place I was living in and I hated going to school. This point on I had lost contact with my best friend in Colorado and I had felt lonely just as I had originally thought I would feel. I had felt this same way all the way to almost between the end of middle school and the start of high school. One day while riding the bus home from school. A guy that sat behind me I said â€Å"hey. † This is the point where I had met my friend Stephen Brinson.Stephen had become almost like a brother to me. He was like a part of my family and even my parents seen it that way as well. This is when things seemed to get better for me, slowly but gradually I would deal with my problems. I no longer felt as lonely as I did before because I had met someone that I could talk to and seemed to have the same interests as me. I have known Stephen for a while now and he is still my best friend today. He is the only person I honestly probably see and talk to most of the time. In my first two years of high school I was not popular at all. In fact, I was probably more of a geek than anything.It was the last two years of high school that I had finally felt a difference. I never realized how many people I knew. I had actually become pretty popular at the end of my high school career. I was known for my talents and nominated for everything. I feel that Stephen was a part of this change because we had made that change together. Stephen was also a pretty big geek in the beginning of high school as well, but we both decided to make this change together. I feel if I have never met him, then that change would have never occurred and I could probably be in a situation that I hate.I learned that it is not about where I go, but the peop le that I do things with. I still today tend to not make a lot of friends, but a few is all I need to be happy. I learned that being made fun of is nothing and it is something that I can overcome. In the end it has made me stronger as a person and I tend to think back at a hard time and laugh about it. I laugh at it because I realize where I am now and I am proud of that alone. My last event is about my sister and mostly my mother. My mother is pretty strict mom and she pushes us very hard especially when it comes to education.The way she pushes my sister and I does not seem very reasonable and at times very unfair. My sister and my mother would argue a lot, but one day it had got to the point where I had come home and my sister had about nine knives that she held up to her neck explaining that she just wanted to kill herself. I remember me sneaking up behind her and taking all of the knives out of her hand to make sure that she would not harm herself. From that point on I tend to k eep a barrier between my mother and I. This barrier is the only one that I can see fit for dealing with her because my mother knows exactly how to tick you off.I do believe that my mother’s ways did help me during school because I had focused and did well many times until the start of high school. In the beginning of high school I tended to rebel against her because I wanted to show her that I did not really care. It was about a year into high school that I realized it was not for my mother, but for myself and that I should actually be doing a whole lot better. My mom did not even attend my high school graduation because she believes I was not high enough ranked. I ranked in the top 10 percentile of my high school, which I thought was pretty good.I do not tell her absolutely anything unless it is completely needed. There is nothing about my personal life that she really knows because she will just argue about how everything I am doing is completely wrong. The barrier is the o nly way I believe that I can enjoy my life and not bother hers. The barrier is not just for myself, but a little part of it is for her as well. It keeps us both sane instead of in a state of constant rage. I believe that I will always have this barrier between her because she will never really sit down and understand why I do the things I do.She does not want to hear what I have to say and just believes that nothing is right compared to her. By creating this barrier between her and I. It has changed me as a person. I realize that I create barriers between other people because I tend to not trust others very quickly. Even when I get into a relationship with a significant other I create some type of barrier and the girl will never understand me completely. I always end up telling them that they just do not get it but I know that because of this barrier I put up. It makes it hard to understand me.I am trying to work on this because I have met someone that I would really love to fully u nderstand me and I fully understand her. It is a work in progress, but I believe in the end it will be worth it. Today, I no longer feel as lonely as I once did. I feel satisfied with how things are going in my life today. I think that I have it all. I might not have the perfect family, but I do love my family. I do not think that anyone has a fairytale. This class has shown me so much and has given me different perspectives on how I can deal with the everyday stress of life.Things just seem a lot better than they did before and I can really only thank myself for this. I feel if I never wanted things to change then they would not have, but I had accepted others with positive inputs in my life and let the ones with negative inputs go away. I thought that this class was an interesting course and that anybody regardless if they are psychology major or minor should take this class because they could also learn something from it as well. It is a different experience than the normal class room, but that is exactly what makes it so good.

How People Grow Reflection Paper by Cloud and Townsend

REFLECTION PAPER OF HOW PEOPLE GROW BOOK BY CLOUD AND TOWNSENDVillatoro-Ramirez Counseling Department Denver Seminary Monte Hasz Psy. D. Elisabeth A. Nesbit, Ph. D. CO 631 01 Career Development and Assessment October 8, 2012 Reading How People Grow (Cloud and Townsend, 2001) has been an interesting experience because the author of this paper’s knowledge about counseling has been transformed in different ways. This reading has forced me to evaluate my own approach to counseling and my spiritual life has been impacted through the reflection about grace and law as well.I appreciate that this book has already been translated to Spanish language, which has given me the opportunity to read faster and better understanding of some ideas because Spanish is my first language. I have already included this book as a required reading to the course of Pastoral Care, which is taught at Denver Seminary’s Hispanic Initiatives because in my personal opinion this book is useful for all pe ople in Christian ministry who wants to know how people grow. Thus, through this paper I will explain my understanding on how people grow according to Cloud and Townsend.In addition, I will critique the book comparing it with two of the systems of integration presented in class by Drs. Nesbit and Hasz. To accomplishing it this paper has been divided in three areas. The first explains my personal understanding about what Cloud and Townsend (2001) say about how people grow. The second area compares this book with two systems of integration, which are Biblical counseling and the Integration approach. Finally, the rest of the paper focuses on reflection and application as a resource for clients.First, I think Cloud and Townsend (2001) use the concept of Christian counseling and discipleship or personal growth in the same way. For them there is not difference between spiritual life and real life (21). The heart of their book presents the idea of working on relational and emotional issues back into the mainstream of spiritual growth (21). It can be seen in the chapter the Paradise lost where Cloud started to describe that at the time that he began his ministry there were four models of how people grow (16).He was clear saying that he saw value in these four models practicing all of them in some degree even though he was inclined for the model of truth which held that the truth would the person free. If the person was not free or some area of life was not working it must be because lacked truth in the life of this person (16). However, and even though Cloud (2001) saw people improve, and prayer, learning Scripture, and repentance were very powerful elements in healing many clinical conditions he realized that something was missing in all these elements (18).He realized that all these spiritual methods did not resolve some problems (18). People were helped but not cured (18). Thus, Cloud (2001) became disillusioned and went for further training and has experience with sincere Christian people who had been very diligent about spiritual growth but without success (19). However, he continued to work in Christian counseling for a few years more when he started to see processes that actually changed people’s lives (19). These processes were not the traditional Christian way to grow that he had learned.These processes involved deep transformations of the soul and there was the spiritual life, where we learned about God and grew in our relationship to him, and then there was the emotional and relational life, where we learned how to solve real-life problems (19). In other words, both Cloud and Townsend (2001) believe that the issues why people come for counseling are not growth issues or counseling issues, but spiritual growth issues (21). In addition, Cloud and Townsend (2001) emphasis that spiritual growth should affect relationship problems, emotional problems, and all other problems of life (22).For them there is no difference between spirit ual life and real life because it is all one. They go through the major categories of Christian doctrine and talk about how each doctrine applies to personal growth (23). Second, about the comparison of this book with two systems of integration, I think in some point Cloud and Townsend (2010) are part of the biblical counseling approach which focus is on the sufficiency of the Scripture for counseling needs.A definite strength for Cloud and Townsend’s book is their use of the Bible and appeal to its authority as well. Contrary to Scott’s presentation of Biblical counseling, Cloud and Townsend (2010) are not directive and sermonic. They are more practical and enlighten the process of how people grow through the emotional and relational life, where they share practical ideas about how to solve real-life problems. In this sense, there is not way to use their method out of the church or Christian environments, which I think it is a weakness of their method.In other words, following this methodology a counselor would not seek the governmental endorsement of a professional license and cannot work in secular counseling settings as a rule. Because Cloud and Townsend (2001) at the beginning of their book expressed through some examples of Christian people facing mental problems in a hospital, I thought that they may lean toward the integration approach looking to secular models for treatment techniques. However, in the rest of their book they did not mention about a different way of treatment more than practical counseling application of the Bible.In addition to this point I can say that it seems to me that Cloud and Townsend (2001) use group therapy as a source of help people even though they say that working in group is an external spiritual source that God has given us to grow. I think to working in a Christian environment, this idea of groups to help people to grow is powerful. However, it seems to me that Cloud and Townsend (2001) did not want to be related to the four methods that were at that time they began to work on counseling.However, this book ends doing almost the same thing that biblical counseling in the sense that does not giving space for secular techniques as a source of help other than the Bible. Cloud and Townsend (2001) differentiate internal and external sources that God has given us to grow (126). For them internal sources are Bible, prayer, and Holy Spirit and external sources are all help that others can gives us as a way of help. In this sense, they talk widely about the work in groups, which I think it may be a secular technique however they did not recognize that.It seems to me that they are ambiguous not telling that they are combining some secular techniques in their work at Christian environments. Finally, because Cloud and Townsend (2001) were clear to say that their desire that their book be practical, and help the reader (I think Christian reader) to understand how the growth process is theological. I agree that in my life they accomplish that desire because there are some areas of application that I can mention that would like to implement in my ministry. First thing that I learned in the reading of â€Å"How people grow† Cloud and Townsend, 2001) was the concept of grace and law. I think in our Christian world we have misunderstood the concept of grace and that is why we live under the idea of a God who is ready to punish us. At least it is what I discovered on my own life through this reading. I like how Cloud and Townsend (2001) explained that grace is something that we cannot give ourselves because it comes from outside of us, as unmerited favor; and we can not do anything to obtain grace (127). It was powerful to me learning that grace is an external source of healing that we can give and receive through the body of Jesus, the church.I appreciate that Cloud and Townsend (2001) remind us that God created us to be in relationship with others so it is through others that people can grow as well. I love this concept because it is contrary with what our individualistic culture says. In addition, this concept is contrary with some teachings that say that people do not need other people at all because Christ alone is sufficient or that his Word or prayer is enough. These teachings actively and directly lead others to not depend on people at all because they think it is wrong.I like how Cloud and Townsend (2001) explained that the Bible teaches that all these things are part of the process, including other people (121). Through the book both of them Cloud and Townsend (2001) emphasized loudly the role of the body in the process of how people grow. In sum, how people grow (Cloud and Townsend, 2001) is a great tool for counselors who work in Christian environments because offers practical ideas about based on the Scripture. Differently to the Biblical counseling approach, Cloud and Townsend (2001) focus on the grace instead of the law.In addition, Clo ud and Townsend (2001) emphasize the role of the body of Jesus as a source of growth in the life of the believer. Cloud and Townsend (2001) is not recommended for counselors who work in a secular settings even though the principles that come directly from the Bible must be applicable in the personal life that any Christian counselor no matter where his or her job’s place be. A new research done in Washington describes the trade deficit with China as a cost to the jobs in the United States of America. This research further found out that millions of jobs were lost due to the low wage workers of China. As the products made by the Chinese companies are cheaper the demand for them is increasing day by day. 5The poor consumers prefer using the Chinese products than that of the Americans; this is because of the lower rate of these products. The prices of clothes and electronics have decreased significantly due to the Chinese companies. The companies of the United States which have not been located in China have suffered major setbacks due to this trade deficit. The garment industry of America has been struck majorly by these blows. Home appliances and electronics are largely being manufactured in China and are being further sold in the United States.Devaluing of a certain currency is done to rectify t he problems one country is facing. It can help one country to get over the debts it is facing and would provide a path to the outsiders (civilians) to take advantage. As said by one of the Chinese man when dollar was devalued "I can travel more frequently and stay longer in my favorite cities as the currency is devalued". It helps in achieving the goals of social and economic integrity. Devaluing a currency can help the country to slim its trade deficit, can create jobs for the locals and can boost the exports. 6A situation arose for the United States in which the trade deficit was increasing every fiscal year and some strict action had to be taken against it. In response to this the United States devalued the currency of dollar to adjust to the monetary problems faced by it. This action taken by the United States is thought to have some effect on the relationship between the United States

The Nmc Code Of Conduct Nursing Essay Nursing is a profession regulated by the Nursing and Midwifery Council (NMC 2008). The NMC is an organisation set up by the Parliament to protect the public by ensuring that nurses and midwives provide high standards of care to their patients. These healthcare professionals are also accountable for their own actions. The body sets standards for education, practice and conduct as well as providing advice for nurses and midwives. The NMC also considers allegations of misconduct or unfitness to practice. Using the case study given, it shall be the authors aim to demonstrate the understanding of the NMC Code of Conduct suggesting ways in which it can be applied to practice. In order to comply with the NMC Code of Conduct of confidentiality, the patient to be discussed in this assignment will be referred to as Mrs X. Furthermore the author will explore the four main principles of the code relating them to issues arising out of the case study. The author will also demonstrate the understan ding of ethical issues arising, analysing and discussing autonomy, non-maleficence, beneficence and justice. The case study refers to an 80 year old woman with a hip fracture, admitted to a hospital ward from a nursing home and urgently added to the operation list. She is bedridden, with severe heart problem and in early stages of Alzheimers disease and appeared to be coherent and lucid as recorded. She agreed to have a hip replacement operation after the consultant explained the procedure. On her way to theatre, she changes her mind and the consultant was informed. The consultant insists on proceeding, citing a busy week ahead and commenting that these elderly confused patients dont know their own mind. According to the NMC Code of Conduct, a healthcare professional has a duty to care and protect the interest of those in their care regardless of age, gender, culture, religious and political beliefs. Mrs X is 80 years old but the professionals still have a duty of care and must protect her interests. An interview was carried out by the medical staff and the patient appeared coherent and lucid but Mrs X has changed her mind on route to theatre. This author will critically examine the procedures that followed. The consultant explained the procedure to the patient who agreed to have the operation. Thompson et al (1994) stated that communication is one of the fundamental aspects in nursing The consultant was informed of the patients decision to change her mind on the operation and responds stating that We will have to proceed. As a nurse one could argue that the consultant should respect this decision as going against it would be breaching the NMC code. Mrs Xs decision to change her mind on the way to theatre, not wanting to go ahead with the operation should be respected. Hope et al (2008) stated that a patients autonomy can result in conflict, raise ethical dilemmas and may not be straight forward. Autonomy is defined as the right to choose or refuse treatment .Beauchamp and Childress (2009). The consultant could also argue that he is working in the best interest of the patient but does this override the patients right to make her own decision? Beauchamp and Childress (2009) stated that individuals views and rights must be respected as long as these individuals thoughts and action do not cause harm to other people. The NMC makes a point of highlighting the point of advocating for patients. In this instance, the nurse faces the ethical dilemma of standing up to the consultant and advocating for the patien t in order to uphold the code. Thompson (2003, cited in Buka, 2008) suggests that ethics is a study of how people behave, what they do, the reasons they give for their actions and the justification behind their decision. The need to maintain professional boundaries as well means that nurses have to raise their points in a manner that does not destabilise the team. Each and every member of the healthcare team must act as the patients advocate and remind or challenge colleagues should they fail to practice according to standards, Hindle and Coates (2011). If any medical team members working with the consultant on Mrs Xs case are not in agreement with his decision to proceed, they should challenge or remind him of the ethical code stipulating that the patients decision must be respected. When healthcare professionals are faced with dilemmas, patients should always come first. Childs et al (2009) states that when considering our actions we are bound by NMC codes, standards and guideline s, for students guidelines set by their training institution by local standards and guidelines within the clinical practice area and by the law of the country. It is unprofessional and unlawful to force treatment on anyone. Although the consultant explained the procedure, one could argue that making information available is different from effective communication. Consent was given the first time but the patient later changed her mind. An exploration for her reason to change her mind should have taken place and at least inform the patient that the operation was going ahead and the reasons for going ahead. The wording used by the consultant could be a concern. We will have to proceed. We have a very busy week ahead; these elderly confused patients dont know their own mind. Carry on as usual. One could interpret that the consultant is suggesting that when people get old, they automatically become confused; which could be stereotyping amounting to discrimination which is against the law. This could be taken to suggest that the consultant is of the assumption that the elderly are confused and dont know whats good for themselves and so should have decisions made for them. Patients are supposed to be treated with dignity, respect and as individuals considering their physical, psychological and social care with decisions made in partnership with clinicians, rather than by clinicians alone according to DOH (2010). Hendnrick (2004) defines consent as the permission given by patient voluntarily, without pressure, force or manipulation or undue influence. The NMC emphasises that healthcare professionals must seek consent from their patients otherwise they might be liable to be charged with assault or battery. The consent could either be in writing or verbal. In the event of a law suit, such documents and discussions can then be used in courts of law. Although Mrs X had given consent for the operation to proceed, health professionals should respect the withdrawal of consent. Proceeding with the planned operation against Mrs Xs wish amounts to violation of her rights and the nurse has a duty to highlight this aspect. The Mental Capacity Act (2005) was established to empower and to protect vulnerable people in making their own decisions. In particular, this was to safeguard those who lack capacity and those who have difficulties in making decisions because of illness, disability and those with mental health problems. The mental capacity act has four main principles of capacity: A person must be assumed to have capacity unless it is proved otherwise. Mrs X should be deemed to have capacity as she was interviewed and appeared coherent and lucid. Any act or decision taken on behalf of someone lacking capacity must be in the persons best interest. The consultant could argue he was working in the best interest of the patient. In the event that Mrs X lacks capacity, an advocate could be appointed to act on her behalf. An unwise decision is not to be taken as a lack of capacity. Even though Mrs X changed her mind and appears to have made an unwise decision, this should not be seen as lack of capacity. Until all practicable steps have been taken to help someone make a decision without success, they cannot be treated as lacking capacity. The consultant did not exhaust all efforts to help Mrs X in her decision making as no interaction took place after she changed her mind. The consultant took it upon himself to make the decision and dismissed Mrs X as an elderly confused patient who does not know her mind. The Mental Capacity Act (2005) has a test for capacity which states that a person lacks capacity if at the material time he is unable to make a decision for himself in relation to the matter because of an impairment of or a disturbance in the functioning of, the mind or brain. It does not matter whether the impairment or disturbance is temporal or permanent (Brammer, 2007). A person is unable to make a decision for himself if he is unable to understand the information relevant to the decision, to retain the information, to use or to weigh up that information as part of the process of making the decision, or to continue the decision (Brammer 2007). Section 3 of the act states that if the patient can retain information relevant to the decision for a short time only, this does not necessarily mean she cannot make a decision. When Mrs X changed her mind the consultant should have respected this decision because she was capable of retaining information for a while, had thought it through and decided she did not want to proceed. The Mental Health Act (1983) covers the reception, care and treatment of mentally disordered persons, the management of their property and other related matters. The act empowers authorities to detain those diagnosed with a mental disorder in hospital or police custody and have their disorder assessed or treated against their wishes, known as sectioning. Mrs X was diagnosed as having early signs of Alzheimers disease. This disease is a form of dementia, a neurologic disease characterized by loss of mental ability severe enough to interfere with normal activities of daily living. It usually occurs in old age, and is marked by a decline in cognitive functions such as remembering, reasoning, and planning. As Alzheimers disease is a progressive illness with no recovery, it is not applicable to use the Mental Health Act (1983) because whether or not treatment is given for the disease, this will not improve the decision making capacity of Mrs X. The General Medical Council clearly stipulates that healthcare professionals ought not to discriminate but should treat those in their care fairly based on their needs. The consultant is going against the GMCs code of conduct when he ignores the patients wish to discontinue with the operation. The GMC emphasises that patients have the right to change their minds on decisions.(ref) Nurses are required by the NMC Code of Conduct to express compassionate attitudes in their careers (Byrne and Byrne 1992). Nurses act as advocates for patients and as such can be described as special and unique to other health care professionals as they spend more time with the patients (Norman and Ryrie 2004). They are expected to develop a nurse-patient relationship which must be kept professional. It is also a nurses duty in accordance with NMC to educate the patient. Mrs X should have been educated and made aware of the advantages and disadvantages of the operation. The principle of non-maleficence is one which seeks to avoid intentional harm. Mrs X does not wish to undergo the procedure so to agree with her wish would be harmful although proceeding may harm any existing relationship between the healthcare professionals and the patient. What then happens if for instance the procedure does not go according to plan? Operations to correct hip fractures in the elderly are common and to abstain from conducting them would result in a lot of pain and discomfort not to mention the immobility issue. It is common knowledge that bedridden elderly patients if not moved regularly will develop pressure sores (Onslow 2005). The principle that requires action which benefits the patient is known as beneficence. To effect such an action sometimes medical professionals have to ignore the wishes of the patient if they can prove the patients incapacity to consent. While respecting the right of Mrs Xs treatment refusal, capacity test should be done to find out if she is capable of making her own decision. If Mrs X lacks capacity, then the medical staff should seek consent from the relatives or Independent advocates (Tingle and Cribb,. 2008). The ethical difficulties are compounded by such cases as the Canadian case of Malette v Schumann. The claimant came to hospital after being involved in a road accident. The doctor went ahead to perform blood transfusion despite the nurse having found a card in her pocket stating that she was a Jehovahs Witness and never to be given a blood transfusion. Later, on recovering the claimant won $20,000 of damages (Tingle and Cribb, 2008). The doctor was charged with battery. Mrs Xs wish not to proceed with the operation may be well founded and give grounds to litigation. The outcome of the operation also plays a major part in determining whether the decision to go ahead and operate is a good one or not. On tacking this assignment l learnt that establishing the patients consent is very vital for any action to be justifiable carried out. The consultant did manage to convince Mrs X to agree to undergo the procedure after talking her through it. He unfortunately could not accept her change of mind sighting her age as the problem. I felt that Mrs X hadnt been given enough time to ponder the idea of undergoing the procedure. She has been admitted to the hospital ward and urgently added to the list. I thought because she was in pain, she was not thinking straight and was pressured into giving consent. Looking back l now feel the consultant wanted the hip fracture operation to proceed as soon as possible as this would in turn ensure speed recovery. Looking at her age, I would like to think that the sooner she got operated on the quicker the recovery. He had the patients interest at heart. At the time l felt team work and better communication would have brought about better decision. The team members should have objected or aired their feelings against the consultants wish to proceed without consent. The positive was that if Mrs X was operated on, the pain would easy and she would then be mobile, which would be good for her heart. Taking the age issue into perspective the sooner she underwent the procedure the sooner she was expected to heal. The negative was that if anything went wrong, bearing in mind Mrs X had severe heart problem, the whole team would be in trouble. When Mrs X changed her mind about undergoing the procedure the issue should have been addressed properly since consent is fundamental in a patients care. A meeting between the medical care professionals to look into the reason of change of plan, if need be, a mental capacity test taken as is warranted under the Medical Health Capacity Act. In nursing the interests of the patients always come first. I think communication is vital in nursing. Communication is very important when dealing with patients in nursing. The consultant did not act as a professional when Mrs X changed her mind that she is not ready for the hip operation. I was not comfortable with his response as it sounded harsh, commanding and unprofessional when he was informed of Mrs X decision I have learnt that team is important in nursing and healthcare professionals should always respect the rights of their patients and consent is at the centre of every action. It has been identified that although the NMC provides guidance and regulates the nursing profession, the onus is on the practitioner to make decisions based on the guidelines. For efficiency in an organization, all the people of the organization must be able to convey their message properly. 3. Performance Appraisal Meeting. The main body of training and development is adults, and the way adults learn and develop is different from the way students do. Adults training has more specific purposes, and is usually more practical, with the aim to solve practical questions. Hence, the organization or department that are responsible for developing training programs need to know about theories of adult learning. In the 1950s, Malcolm Knowles developed a new theory for adult learning, which is called Andragogy. Andragogy is in the context of adult learners which is contrasted with definition of childs learning (called pedagogical learning). Michael (2007) suggests that the central idea in the context of adult learning is that it is only after convincing oneself of the rationale of learning that an adult will decide to (and be able to) learn. Learning organization enhances organizations value by new ideas and information, instead of material production. 4.2 Training and Development in Infosys Training and development is mainly carried out in Infosys by its HR department. Currently the whole company has more than 20,000 employees, and training and development has always been a key function of all HR department in all branches. The company is willing to provide excellent training opportunities for its employees. Though the leading department of training and development in Infosys is its HR department, other departments also take part in and collaborate with HR department. Training and development is carried out by HR personals according to various requirements of different departments which are located in different geographic locations. There are training and development specialists who are responsible for certain training contents, such as technology, business knowledge, company policies, marketing etc. HR department plans yearly training and development schedule together with experts from other departments, and is responsible for carrying out these training schedules. Training courses in Infosys are presented by subject matter experts(SMEs) in the company or by experts from other organizations and universities. Training specialists need to record teachers performance during the training sessions. After training sessions, feedback from employees are collected and analyzed, in order to better the quality and efficiency of the companys training programs. The company creates its training policies which is part of the companys HR policies. And violation of training policies will be reported to the employees reporting manager. This ensures the quality of training and development in Infosys. Training and development plays an important function in Infosys daily operation and strategy development. The HR department divides training and development mainly into four levels. The first level is for senior management and candidate senior management; the second level is for normal employees and normal managers; the third level is for first-line sales personals; the forth level is for new hires and fresh graduates. According to the needs and requirements of different levels, training sessions are designed by different teams. Garavan, Costine, and Heraty (1995) notes that training and development (TD) encompasses three main activities: training, education, and development and that these ideas are often considered to be synonymous. In Infosys, training and development can also be view from these three parts: 1. Training: This activity is both focused on the job tasks that an employee currently handles. It is usually conducted through the form of classroom training, on-the-job training, and competition in Infosys. 2. Infosys provides books and self-pace learning courses for such activity, and employees have more flexibility to study on their own paces and they can choose to learn during work time or at weekends. 3. Employees performance is evaluated according to several aspects, and among these aspects, employees training and development is taken into consideration and could affect employees final performance appraisal. Infosys require its regular employees to take at least five training sessions in one years, to make sure every employee is equipped with latest skills and knowledge so that they can better make contribution to the company and help the company achieve its strategy goals. Employees need to complete the Job Skills Workbook which includes the skills that employees must have to complete their job tasks. This workbook is used as the basis for the performance evaluation. Both employees and managers will have to agree on the type of skills that are included in the Job Skills Workbook and which training level the employee needs to take. Besides, a group of employees can work together to identify their individual and collective training and development goals. Then these goals will be disc ussed with training specialists and their managers, and HR department will try to arrange according training programs for these employees. This, without a doubt, strengthens the companys capability and improves productivity. In Infosys, several activities are undertaken each year in order to develop key skill elements identified. These could include new work experiences, responsibilities and challenges, mentoring, coaching or more formal training. Once identified, these elements will also be included in the performance evaluation of employees. So training and development help identify new skills and responsibilities of employees, and performance management functions as a monitor and evaluation mechanism for these new training and development needs. After training sessions, the manager or team leader needs to provide the employee with feedback about their current performance and their goals for the next agreed time period which is usually one year. Finally, at the end of the agreed time period the employee needs to once again talk with his or her manager or team leader and discuss how they have performed and whether there are any changes in their training and development schedule. 5.2 Strength and Weakness of the integration of Performance Management and Training and Development in Infosys Such integration of Performance Management and Training and Development in Infosys has several strengthens: 1. It creates a multi-level mechanism for performance management. By discussing with managers about what kind of skills and performance employees agree to achieve in advance, employees can know clearly what both them and their managers expect from them. Employees can attend various training programs to reach their goals and use what they have learned in their daily jobs, thus improving their productivity and job performance. 2. Employees have more choices about what they want to develop in Infosys and what their career paths are. The value of Infosys is to make every customer satisfied. It is only through excellent work that the company can win high trust from customers. When Performance Management and Training and Development work closely together in Infosys, each employee can have the choice to decide which career path they like to take, such as technical experts, people managers or delivery managers etc. With a clear target and purpose in mind, employees are more motivated to take training and development programs provided by the company and their learning results are later directly reflected in performance evaluation in performance management of the company. On the other hand, however, there are some weakness of the integration of Performance Management and Training and Development in Infosys: 1. It puts too much focus on job skills but ignores management skills. Infosys is a large international company and its services in IT industry are among the best. The company focuses too much on the skills of its first-line employees, and almost 80% of the training and development program are about job skills. The performance evaluation of employees also mainly evaluates what new skills employees have learned through such training programs and how they perform. But in the long run, management skills are also required by employees as some of them are potential managers in the future. 2. The training sessions in Infosys are only for regular employees, not for contractors. So the result is that contractors, that occupy more than 40% of all employees, cannot receive proper training in time. Because contractors are not allowed to participate in the training and development activities, when they discuss with their managers about their performance evaluation, there is no section for training and development. This hinders contractors from developing their skills and could probably hinder the companys strategy development in the long run. 6. Recommendations Based on the strengths and weaknesses of the integration of Performance Management and Training and Development in Infosys, these recommendations could be taken into consideration when the management makes future decisions. 1. Management skills should also be included in Infosys training and development activities, and more first-line employee should have access to it. These employees would become managers in the future, and providing proper training programs on management skills would enhance their productivity and make their work more efficient and effective. 2. Training and development should cover contractors. In Infosys, there are two types of employees: regular employees and contractors. Currently most training and development programs are for regular employees only. By providing contractors the same training and development opportunities, the overall workforce of Infosys can be strengthen, and the companys productivity on the whole can be enhanced. 7. Muscle tissue contains phosphocreatinine which is converted to creatinine by a nonenzymatic process. This spontaneous degradation occurs at a rather consistent rate (Merck, 1991). Causes of increases of both BUN and creatinine can be divided into three major categoriesprerenal, renal, and postrenal. Prerenal causes include heart disease, hypoadrenocorticism and shock. Postrenal causes include urethral obstruction or lacerations of the ureter, bladder, or urethra. True renal disease from glomerular, tubular, or interstitial dysfunction raises BUN and creatinine levels when over 70% of the nephrons become nonfunctional (Sodikoff, 1995).
